Actually this thing exist!, you just download COLORS! for NDS and voila, you have a wacom cintiq in your pocket!

The Nintendo DS has a pressure-sensitive screen. I don't think there are any games that actually use this (telling children to hit the screen HARDER to achieve something is not a good plan) but homebrew programs make good use of it.

It lacks the 'second button' of your design, but as you're likely holding it in your left hand you can just use the L trigger for that, and then whatever pen you want.

I should start bringing my DS to work again...
